The staff at Tas on The Cut, who, when I ordered a glass of juice, established that it was to be split between the kids, checked that therefore that would mean that I wouldn't want ice in it, and provided it, unasked, divided into two small glasses with straws. We ordered the set mezze for four between the five of us (3 adults, 2 smalls) and they were careful to give us one extra of the indivisible items. Finally - and this is the clincher, and something that no other restaurant has ever done in my experience - at the end of the meals they asked if I would like them to take the kids' cutlery that I'd brought for them to use and wash it up - which I did, so they took it away and brought it back swiftly, clean and wrapped in a paper napkin.
